Health Affairs Round Table Application for Membership

The membership application below is for the 2007 Calendar Year.  Each Standard Membership entitles you to have two (2) representatives per organization.  Additional members may join for $10.00 each.

As a member of HART, you are entitled to attend 6 Networking Luncheon Meetings throughout the year with many of the movers and shakers in the senior focused business community in Wake County.  You have access to the other members of HART to announce your seminars, special events, meetings, etc.

Please Note:  Submission of the form below serves only as your intention to join HART.  We will take no action except to note your interest. To become a member, payment must be received.

MEETING FEES
Our meetings are luncheon meetings and the cost for this meeting - including lunch is $15 for current members only.  This fee includes your speakers, networking and meal.  Non-members who would like to attend the meeting will be allowed to attend, but will be required to pay $20 at the time of the meeting.

MEMBERSHIP DUES
Dues are collected no later than January 31 for the current year.  If you wish to attend meetings after January 31st, you may do so, however higher rates may apply.  The scheduled fees for membership of 2 individuals is as follows:
Organization/Corporation        $55.00
Sole Proprietor                        $35.00
Additional Members:               $10.00 each
Luncheon Fees                        $15.00/member or $20.00/guest
